This question came up in a game vs Mister Weasely. I believe the Balrog card says something in the line of "If the Witch-King is not in play..." so at first you would think it's Okay to bring him if the WK is dead.
But I remember reading somewhere that if the Shadow player muster Saruman and he then dies, the Free Peeps can still muster Gandalf the White (A minion is considered to be "in play" for that purpose). And the WK cards are saying something like "If the other version of the WK is not in play" and we all know you can't bring the alternate version if the first one is dead.
So what about the Balrog vs WK?

There is a difference.
If the WK is dead you can muster the Balrog.
In the case of mustering GtW after Saruman has enterd and died it is the fact that a (any) Minion has been mustered that allows for GtW to be mustered.

The text on all 3 cards is the same so, apart from plain common sense (It's obvious that Witch King #2 can't come out if Witch King #1 is dead), is there anywhere these things are clarified?

No, only that the expansion clearly states that the Balrog can be mustered as long as the WK isn't on the board.
The Gandalf - Saruman situation is a clarification to solve a very specific situation (Saruman being mustered and killed before GtW is mustered).
