I'm confused by the exact conditions for being able to bring out gandalf the white. Is it:
(Gandalf is dead) OR (gandalf left fellowship AND any minion in play),
or is it:
(Gandalf is dead or left fellowship) AND and minion in play.
Also, I want to be sure, Saruman does othingy as a 'Sauron minion' for this, correct?

In order to bring in Gandalf the White the following conditions must be fulfilled:
Gandalf the Grey must have left the Fellowship, either as a casualty or by separating him using a Character Action die result (or a Will of the West).
Any one of the Shadow's Minions must be in play (Saruman, the Witch-King or the Mouth of Sauron)
If these both conditions are fulfilled Gandalf the White can enter play if the FP player uses a Will of the West die result.

I know this seems like a bit of a silly question, but if Gandalf the White dies, can he be brought back?

If Gandalf is killed after having changed into his more powerful version he is dead for the rest of the game.
